Cogir of Carolina Bay
One campus in Wilmington. Two distinct buildings. Every level of care you may need.
A full continuum of care, on one campus
Cogir of Carolina Bay is a single campus in Wilmington, North Carolina, made up of two buildings that work as one community. Carolina Bay at Autumn Hall by Cogir is our Independent Living building. Bradley Creek at Carolina Bay by Cogir is home to Assisted Living, Memory Care, Skilled Nursing and Rehabilitation.
That means you never have to start over somewhere new. If your needs change, your care changes with you, and the friends, staff and routines you love stay right where they are.

Move forward without moving away
Most families end up piecing senior living together. One community for an active retirement, another for daily support, and a third for nursing care. Each move means new faces, new paperwork and a new drive across town.
Carolina Bay was built to end that. Both of our buildings sit on the same campus off Carolina Bay Drive, minutes from downtown Wilmington and Wrightsville Beach. Couples with different needs can stay close. Families visit one address. Care teams share one set of notes.
How it works
Independent Living
Independent Living at Carolina Bay is for retirees who want more out of a day, not less. Your apartment is yours. Your schedule is yours. What changes is everything you no longer have to do, from cooking and cleaning to yard work and home repairs.
Residents of Carolina Bay at Autumn Hall enjoy chef-prepared dining, a full calendar of programs, fitness and wellness classes, and a walkable Autumn Hall setting close to shopping, healthcare and the coast. And because Bradley Creek is right here on campus, you have peace of mind built in from day one.
Care services
Bradley Creek at Carolina Bay by Cogir is our care building, just steps away on the same campus. Assisted Living, Memory Care, Skilled Nursing and Rehabilitation all live under one roof, staffed around the clock by licensed nurses and caregivers.
Having every level in one building matters. A resident recovering from surgery can rehab here and go home here. A spouse who needs memory care stays minutes from the partner who visits daily. Care levels can shift without another search, another tour or another move across Wilmington.
Living Options

Independent Living
Private apartments, restaurant-style dining, housekeeping and a calendar built around what residents actually want to do. Independent Living is for people who are doing well on their own and would simply rather spend their time on something better than chores.

Assisted Living
Help with the tasks that have gotten harder, from bathing and dressing to medication reminders, delivered in a way that protects dignity and independence. Care plans are personal and adjust as needs change. You choose the lifestyle. We provide the support.

Memory Care
Our Revere Memory Care approach is relationship based. Team members learn each resident’s history, habits and comforts, then build days around them. Secure surroundings, personalized care plans and individual attention help residents living with Alzheimer’s or dementia stay engaged and calm.

Skilled Nursing & Rehabilitation
Skilled nursing and rehabilitation provide personalized care and support for those recovering from an illness, injury, surgery or hospital stay, as well as individuals with ongoing medical needs. With skilled nursing care and rehabilitation therapies, individuals can receive the support they need to regain strength, improve mobility and maintain greater independence.

After visiting 3CCRC’s in the Wilmington area, one of the many reasons we chose Carolina Bay was the incredible staff, including Samantha Johnson and Catherine Lea. After owning a business for over 35 years, I recognize customer service , integrity, and communication skills and Carolina Bay staff represented top notch high level responses to all our questions that gave us the utmost confidence that our decision to relocate to Carolina Bay will ensure our future to be all that we have dreamed possible.
